The entrance to the Bedlam dungeon can be found inside the Bloodletters guild in Umbra. Using the iron maiden in the corner of the guild lets you inside the dungeon.
In order to enter the dungeon you must first talk to the Mistress of Admissions who can be found wandering the area.
